To apply to the NBCHT for membership and testing

• Please download the NBCHT Handbook. Read it entirely, then fill out the application on the last page.

• Ensure that you sign the application and indicate the type of equipment that you are using.

• The equipment must be registered with the FDA and you must provide a serial number that can be verified.

Once the application is received, verified, and processed; you will then be sent an authorization to test. Please comply with the instructions written on your test authorization.
